Latest income Tax Return and audited financial statements for preceding year of operation & Mayor’s Permit-preceding year. (Not applicable for new establishment).
List of officials and employees and their respective designation (a. Managerial and b. Rank and File) and Bio-Data. For Alien personnel, valid visa from Commission on Immigration and Deportation, labor Permit from DOLE.
Board Resolution designating person authorized to sign and transact business with the City Tourism Office.
Comprehensive General Liability Insurance (validity shall be co-terminus with that of license): See Ordinance
Schedule of Current Room Rates, Food & Beverage Menu & latest Brochure.
Registration Fee : Php2,500.00
Annual Supervision : Php 800.00
Such other papers or documents as may be required such as Tourist Arrivals Report and Staff Training requirements.
Lifeguard and Security (Original Certificate of Training if beach resort / with swimming pool)
