  • List of officials and employees and their respective designation
  • Price List of Products.
  • For Alien personnel, valid visa from the Commission on Immigration and Deportation and Department of Labor and Employment.
  • Payment and prescribed fees: Annual Supervision Fee: Php 375.00.
  • Such other papers or documents as may be required pertinent laws, rules and regulations and relevant local ordinances.
