Philhealth Certification For Mayor’s Permit

SELF-EMPLOYED :

  • Payment of at least one quarter for calendar year 2011
  • If owner of business is employed:
  1. company ID and proof of remittance to PhilHealth
  2. if with employees, requirements for the employed sector will apply
  • If owner is a Life Time Member:
  1. Life Time Member ID
  2. if with employees, requirements for the employed sector will apply
  • If member is under the Indigent Program:
  1. valid PhilHealth ID

 

EMPLOYED SECTOR :

Palawan based:

  • Old establishments:
  1. Complete reports for the year 2010
  2. if establishment is not yet registered, submit registration forms, pay applicable premiums and submit complete reports
  • New establishments:
  1. register the business establishment
  • Centrally remitting:
  1. Valid Certificate of Good Payment Standing issued to Head Office of the establishment (date of issuance of certification should be year 2011)
  2. list of employees assigned in Palawan.
