• Latest Income Tax Return and audited financial statements for the preceding year of operation (Not applicable for new establishment). Mayor’s Permit (p. year)
• List of all officials and employees and their respective designation and Bio-Data.
• For alien personnel, valid visa from the Commission on Immigration and Deportation and Department of Labor
• Registration Fee Php 2,000.00
• Annual Supervision Fee: Php 600.00
• Schedule of current Room Rates F & B Menu & Latest Brochure.
• Such other papers or documents as may be required pertinent laws, rules and regulations and relevant local ordinances.

 
Pension (With beach resort / swimming pool)

• DOT accreditation
• If corporation or partnership, copy Articles of Incorporation/Partnership & its By-Laws; if Single Proprietorship, Business Name Certificate (Not applicable if renewal, unless there are amendments;.
• Latest Income Tax Return and audited financial statements for preceding year of operation & Mayor’s Permit – preceding year ( Not applicable for new establishment)
• List of officials and employees and their respective designation (a. Managerial and b. Rank and File) and Bio-Data. For alien personnel, valid visa from the Commission on Immigration & Deportation, Labor Permit from DOLE.
• Board Resolution designating person authorized to sign and transact business with the City Tourism Office.
• Comprehensive General Liability Insurance (validity shall be co-terminus with that of license): AAA Resort: Php 100,000.00; AAResort Php50,000.00; A Resort:Php 20,000.00.
• Lifeguard and Security (Original Certificate of Training if beach resort / with swimming pool)
