1.9.2   APPLICATION FOR MARRIAGE AND AFFIDAVIT OF MAN AND WOMAN WHO HAVE LIVED TOGETHER FOR FIVE YEARS AND ABOVE

1.10      Monitor and evaluate the conduct of Barangay Civil Registration System (BCRS) among the  66 barangays of the city as to its usefulness and effectivity.

1.11.    Conduct distribution of registered birth certificates generated from mobile civil registration.

1.12      Continued the IEC activities of this office on the importance of civil registration.

1.13     Give advice as to procedures and requirements for clients encountering problems for filing petitions for correction of clerical error and change of first name in their birth certificates as well as legitimation process of their children and filing of affidavit to Use the Surname of the Father (AUSF) pursuant to R.A. 9255.

 

II. PROBLEMS ENCOUNTERED:

2.1          Lack of manpower.

2.2          Insufficient budget for supplies and materials and capital outlay.

2.3          Much delayed payment of disbursement vouchers for acquired in advance civil registryforms from the NSO Provincial Office.
